Well depends on if they use box scanners primarily. Might be able to get past the eye of LV bouncers, but if they're higher end type places I'd imagine they're using a box scanner.

Probably because people were using their real DL number or formatting the number wrong and then complaining.
Even when they did have it, I always left it blank. Just finish the order you'll be fine

Once a barcode is made, that's that. It's much more likely that the places you go to changed what they use to check IDs. If you can figure out what they're using we can know for sure

Update: OIS More States Pass New BCS —
I'd imagine previous barcodes were pretty generic and the BCS app simply checked for that generic stuff. But with the update it likely checked for things that were more unique to each state that past barcodes didn't include
RI IDs are generally thinner. You should see a real NC ID. If you're not used to thin IDs you'd think it's fake. As for the CO ID wtf did you do? Bend it from corner to corner? I have like 4 COs from him and none of them have snapped in half.

It sounds like you ran into a box scanner or patronscan. Both of these scanners are foolproof against fakes from ANY VENDOR and it's on you for not knowing that fakes don't pass these high tech scanners. Sorry.
Just bc it doesn't scan on one app or at a specific store does not mean it " stopped scanning " no one has a magic switch that makes it " stop scanning " unless they all simultaneously updated their software which will never happen
