OIS question - nCA filling in street name ( self )

Hey I'm a noob when it comes to all of this, but do I need to shorten street names? Writing 626 SAN ROQUE RD, instead of ROAD, etc?

Will the Driver Licence no also be automatically filled in or do I have to make up one?

Do I need all 9 digits for the zip code? Or is the first 5 enough?
